| FIRST TIME AND RENEWAL PASSPORTS

Requirements for Adults and Minors
|
| Complete the passport application
form and note the following instructions:
- Original Birth Certificate or Adoption Certificate MUST
be presented with your expired or current passport (with twelve months
validity or less).
- Two (2) identical COLOUR photographs one (1) of which
should be certified by the same Certifying Official (listed at 4.1)
who signs Section F. Features in photographs must not be too dark or
light. There should be no shadows in background or glare in face. Both
ears must be visible. A side profile is unacceptable. No head covering
or headbands, unless for religious purposes. No tinted lenses. No machine
photographs
- Marriage Certificate where applicable (even if you are applying in
your maiden name). Decree Absolute where applicable, Death Certificate
of spouse where applicable.
- Certificate of citizenship or naturalisation or registration document,
if you were born abroad.
- Please submit four (4) photographs if certification application is
being submitted at the same time (two should then be certified).
- In the case of a minor (17 years and below), the parent or guardian
must provide their passport and residence care order or letter of consent
with copy of first five pages of passport of parent, witnessed/certified
by a Notary Public or a Justice of the Peace.

| Renewal applicants can send in
their application by registered mail. Incomplete applications will be
returned at a cost of £5. |

| REPLACEMENT
PASSPORT (LOST, STOLEN, DESTROYED) ADULTS AND MINORS |
| Complete the passport application
form and note the following instructions:
- Submit a written report from your nearest police station or a Home
Office Letter regarding the loss of your passport at their offices.
If your passport has been damaged or destroyed, you must submit an explanation
in writing.
- Original Birth Certificate or Adoption certificate must be presented
with other supporting documentary evidence
- Photographic ID (not your photographs)
- Two (2) identical COLOUR photographs, one (1) of which should be certified
by the same Certifying Official (listed at 4.1) who signs Section F.

- Jamaican driver’s licence, Cancelled passport, National voter’s
identification card, Identification indicating permanent residency,
Passport from country of residence, Citizenship Certificate from country
of residence, Driver’s licence from country of residence, Provisional
driver’s licence, Identification from a school or workplace.

| NOTIFICATION OF NEW ARRANGEMENTS FOR THE
ISSUE OF JAMAICAN PASSPORTS |
| The issuance of new machine readable
passports commenced on 3 September 2001 at the Immigration, Citizenship
and Passport Division of the Ministry of National Security and Justice,
which is now located at 25 Constant Spring Road, Kingston 10, Jamaica.
The new machine-readable passports are now only being issued in Kingston,
in respect of applications which are submitted directly to the Passport
Office.
In view of the above, and to facilitate the phasing in of the new Jamaican
passports, the following transitional arrangements are now in place at
the Jamaican High Commission for:
(1) the extension of validity of passports; and
(2) the issue of interim passports

| Extensions of validity of
Passports
|
| Interim Passports
An interim passport (old type), valid for one year will be issued only
to the following persons:
- Holders of 10-year passports which expired on or before 31 August
2001;
- Persons who are applying for a passport for the first time and who
meet the criteria for Jamaican citizenship;
- Persons who were under 16 years of age when their passports were initially
issued; and
- Holders of valid passports - the pages of which have been exhausted.
Documents required:
a) Completed Passport Application Form A (or Form B, for children under
16)
b) Two (2) photographs. (Photo service available at the High Commission
at cost of £5.00 per set). One photograph to be certified by person
listed (see note 2 on Form A). The same person to certify Section 9
of the form. If applicant has a previous passport WHICH IDENTIFIES
HIM/HER, and makes application in person, certification may
be waived if the details are unchanged.
c) ORIGINAL birth/marriage certificate(s), Final Decree of Divorce
(Decree Absolute) if applicable.
d) You MUST sign your name on the pink slip – DO NOT PRINT! You
must sign the Declaration at Section 8. Both signatures should be the
same.
e) Jamaican passport
f) Fee of £15.00 (Lost, stolen, damaged passport fee £30.00).

Note:
ALL PERSONS SEEKING TO REPLACE LOST, STOLEN OR DAMAGED PASSPORTS
MUST ATTEND THIS OFFICE, bringing police
report, birth certificate and at least two (2) additional forms
of identification, i.e. medical card, driver’s licence, work ID,
etc. and three (3) photographs.
ALL FIRST TIME APPLICANTS MUST ATTEND THIS OFFICE WITH ID,
i.e. British Passport (if born in the UK). Please note below for the
requirements in respect of persons born outside
of Jamaica of Jamaican parentage.

PERSONS BORN OUTSIDE OF JAMAICA OF JAMAICAN PARENTAGE

| On the basis of the amendment to the
Jamaican Nationality Act, an application for Certification as a Citizen
of Jamaica can be made by persons born outside of Jamaica to Jamaican parents.
The procedure is as follows:
Letter of application to be sent to the Ministry of National
Security, 25 Constant Spring Road, Kingston 10, Jamaica, via
the High Commission, and supported by the following documents:
- If born in wedlock to a Jamaican father:
a) applicant’s full birth certificate (showing parent’s
names)
b) father’s birth certificate and Jamaican passport
c) parent’s marriage certificate
- If born out of wedlock to a Jamaican mother:
a) applicant’s full birth certificate
b) mother’s birth certificate and Jamaican passport
- If one parent is Jamaica, but applicant falls outside of
cases above
a) the applicant’s full birth certificate (and passport, if
applicable)
b) (Jamaican) parent’s birth certificate and Jamaican passport
c) parent’s marriage certificate (if applicable)
ALL APPLICATIONS SHOULD BE ACCOMPANIED BY TWO PASSPORT-SIZED
PHOTOS (CERTIFIED BY THE HIGH COMMISSION) AND POLICE REPORT (CHARACTER
REFERENCE) FOR APPLICANTS OVER 16 YEARS OLD.
On the basis of the verified documents submitted, the Ministry of National
Security would issue a Letter of Certification of Citizenship. The said
letter is required to apply for a Jamaican passport.
Please allow between 6 months to one year for the processing of applications
for Certification.
