- MACHINE PHOTOS (PHOTO BOOTH ) are NOT ACCEPTED.

In the absence of an acceptable government issued photo ID, a Declaration of Identity (DOI) MUST be completed by a biological parent or adoptive parent, or older relative or siblings. Declarants MUST have a valid government issued ID; and biological relatives or siblings MUST present a birth certificate to show the familial connection. If the parent making the declaration is the father then the father’s name must be on the applicant’s birth certificate. The same is required if the siblings share the same father.
